I used to teach in Ohio and I can tell you that no such law exists.

If a teacher tells a child NO it's because they are about to give out important directions or things they don't want anyone to miss in the class OR because the child in question abused their privilege by going to the bathroom and disappearing for a long time or getting into trouble doing so, but no such law exists.

Most teachers that I knew as colleagues would schedule a mid morning break, one right before lunch, one after lunch and again mid afternoon so there was no reason to need to go other times. In middle school they usually are old enough to know they need a bathroom and will go before class with a hall pass or before, during or after lunch.

No such law - in fact there may be some case law to suggest the opposite. A highly rated lawyer on Yahoo local would be willing to answer your question directly even through email.

I don't think there is a law that says she has to let him go to the bathroom 50 times a day. Your son/daughter may have been caught playing in the bathroom on more than one occasion and she may limit him to the amount of times he can go.

You have to keep in mind that when a child goes to the bathroom it kinda disrupts the class and if he is going in there to play rather than to actually use it the teacher may have told your child that he/she can only go 2 or 3 times a day.

He may have abused his right to go to the bathroom.

There is no law in any state about children's rights over their own bodily functions. Once a child goes through the schoolhouse door, they lose a great many rights we take for granted.

All the best.

No. If a kid's gotta go to the bathroom, then they should be allowed to go to the bathroom.
