i lost my 10 year old daughters shot records in the process of moving into our new house...she will need them to attend school this fall..how will i go about getting another copy

All I had to do for my son for his daycare is call his doctor. They faxed me a copy right away. There was not a fee, but there may be one if you need a hard copy sent to you, especially in another state.

When you enroll in the new school the old school will send a copy of her shot records along with all the other records... Just tell the new school what happened

Call the doctors office or clinic where she had the shots. They can send you a copy. They may charge you for it since you lost the one they gave you, but you should be able to get a copy of them.

More likely you lost a COPY of her shot records. Go to her doctor or a hospital. Show proper identification and ask for a copy. Her information can easily be transferred from hospital to hospital.

so your usual practitioner is an particularly undesirable word!!! so my twins have been 5-5 and 5-15 while they have been born, at 18 and 18-3/4 long. they're 3 months previous weighing 12 pounds each and every now at approximately 22 inches long for the two and MY pediatrician, who has been my pediatrician for 10 years, says they're doing fantastically, and not basically that, they at the instant are not solely breastfed! push aside that moron's suggestion. no longer basically THAT, yet ( oh see, now i'm all fired up FOR you!! LOL) breastfed toddlers will lose an excellent form of fat while they regulate into cellular. Please. that's unlike you're sitting at domicile doing no longer something yet stress feeding the lil princess. quite! some human beings.... even the medically knowledgeable ones. * vast eye roll* basically proceed to feed on call for. whether that's greater advantageous than each and every 2 hours or so, i could be susceptible to fret, even yet it ISN"T, so THERE! LOL

Her doctor (or former doctor) will send you a copy. There is usually a fee for this service.

Call her doctor's office. They can even fax them to the school you are enrolling her in.
